Athletic Participation Requirements

Before participating in any athletic activity at Bishop Eustace Preparatory School—including tryouts, practices, scrimmages, or interscholastic competition—each student-athlete must complete and submit all required athletic participation forms (links below): Bishop Eustace Compliance Form, New Jersey Health History Update Form, Bishop Eustace Code of Conduct Form, Athletics Permission Form, Athletics Physical Form

Completed and signed forms should be emailed to athleticforms@eustace.org before a student is permitted to participate.
